Our on-campus Health and Rehabilitation Center offers an incredible range of services for people needing Skilled Nursing and Rehabilitation services. Our rehabilitation therapists are certified in an advanced therapy program called Accelerated Care Plus.© ACP is the leading provider of integrated clinical programs, therapeutic modalities, and products and services for geriatric rehabilitation.

Pathways to Progress: Short-Term Skilled Nursing Care

For individuals who are in need of short-term rehabilitation and/or skilled nursing care, we employ the Pathways to Progress sub-acute model of care. Pathways focuses on helping residents progress in their transition from hospital to home. Care is provided in both private and semi-private rooms. Pathways is Medicare Certified and licensed to accept coverage my HMOs and private insurance.

Long-Term Skilled Nursing Care

The Meadows also offers compassionate long-term care for residents in a comfortable, homelike environment.
